What medical records should I prepare?

A clear checklist to help hospitals review your case efficiently and avoid preventable delays.

Clinical summary

Include diagnosis, major symptoms, previous treatments, allergies and current medication in a short chronological summary.

Reports and imaging

Prepare recent laboratory results, imaging reports and original DICOM files where available. A screenshot is rarely a substitute for a complete scan.

Pathology information

For oncology enquiries, the hospital may request pathology reports, molecular results or physical slides. Do not ship original materials before receiving instructions.

Translation

Use consistent names, dates and terminology. Preserve the original document alongside every translation.
