Describe the current functional question
Explain the injury or diagnosis, affected side, symptoms, mobility limits, prior operations, therapies, medicines and the question you want the hospital to address. Include dates; do not rely solely on a procedure name or an informal description.
Organize reports and images
Provide recent X-ray, CT or MRI reports and underlying images if available, together with operative reports and rehabilitation notes where relevant. Ask the named department which files it can access and whether translations are needed.
Plan for an in-person assessment
The team may need a physical examination, new imaging or measurements before discussing options. That is part of clinical responsibility, not proof that a procedure is approved or that prior care was inadequate.
Share mobility needs early
Tell the hospital and any support provider about wheelchair use, walking aids, stairs, companion needs and transport constraints. This helps plan a campus visit safely without turning practical support into a clinical recommendation.
